This is a dental discount plan and is NOT dental insurance.

Guidelines/Exclusions/Limitations:

• Enrollment date begins on the date when enrollment fee is paid in full and expires after 365 days.

• Annual enrollment/renewal fees must be paid in full on/or before each patient’s first appointment.

• This plan is strictly valid at our office.

• Not transferable to another party or uncovered family member.

• All membership fees are non-refundable, even if the member does not utilize the dental plan.

• Plan specifications and annual fees are subject to change without notice.

 

This plan cannot be used:

• In conjunction with other dental insurances or benefits. You may enroll and use this plan when a procedure is not covered by your dental insurance.

• For referrals to specialists.

• For Invisalign and orthodontic treatments.

• For treatment which, in the sole opinion of the treating dentist, lies outside the realm of his/her capability.

• For hospitalizations or hospital charges of any kind.

• For dental care for injuries covered under worker’s compensation.
